Please complete or update the online health history prior to arrival for your appointment. It is located under the appointment data on the portal homepage. Failure to do so causes lengthy delays and may result in rescheduling.
We ask that reschedules and cancellations be made as far in advance as possible, but with at least twenty-four hours notice. We reserve the right to charge $75.00 for missed appointments or same day cancellations/reschedules. There is a $150.00 charge for rescheduling or cancelling surgeries with less than two weeks notice.
Hospitalization and Emergencies
All hospital-based procedures, surgeries and deliveries are performed at Rex Hospital. You may obtain additional information about Rex and the Family Birth Center by visiting www.rexhealth.com.

If you have an urgent medical problem after office hours, please call our main office number, 919-876-9797. Our answering service will forward your information to the physician on call. Please speak slowly and answer all prompts. You may expect your call to be returned promptly. In the rare event that you are unable to reach the answering service please call the Family Birth Center at Rex Hospital.
Physicians are on call to handle medical emergencies. A service fee of $25 will be billed for non-urgent calls made after hours.
Insurance and Billing
Please bring a picture ID, your current insurance card and a credit/debit/HSA card to your appointment. Co-pays, deductibles and co-insurance are due at the time of service. We will file with your insurance company but, require that a credit card be placed on file to be used for the portion your insurance requires you to pay.
We are contracted with most major insurance carriers but suggest that you contact your insurance company if you are unsure of your coverage or network benefits. It is our policy to collect past due balances prior to scheduling/providing routine services. Statement charges and late fees may be applied to past due balances. If you have a problem paying your bill please contact our patient account representative. Payment plans are available if made in advance. We are always willing to work with anyone who shows good faith in meeting financial responsibilities.
We accept cash, checks, money orders, Mastercard and VISA. You may also pay your bill online via the patient portal. There is a $35 fee for returned checks.
Please try to anticipate the need for medication. Refill requests should be made at least one week before your medication runs out. The most expeditious method to obtain a refill is to CALL YOUR PHARMACY FIRST. The pharmacy will submit an electronic request to your physician with all the details required for processing. This will reduce wait time and phone tag between you, our office and the pharmacy. Please allow 48 hours for your request to be processed and then check with your pharmacy.
Test Results and Referral Appointments
If your physician has ordered routine lab tests, x-rays or specialty consults, please allow seven business days for our staff to contact you with results or referral information. Pap smear results may take longer. You will be advised of normal results by mail. A physician will contact you by telephone if there is a concern or abnormal result. Some test results will be reviewed by a follow-up appointment. Please be sure to provide us with current contact information. Do not hesitate to call us with any questions about lab results.
