Please bring a picture ID, your current insurance card and a valid credit card to your appointment. Co-pays, deductibles and co-insurance are due at the time of service. We will file with your insurance company but, require that a credit card be placed on file to be used for the portion your insurance requires you to pay.
We are contracted with most major insurance carriers but suggest that you contact your insurance company if you are unsure of your coverage or network benefits. It is our policy to collect past due balances prior to scheduling/providing routine services. Statement charges and late fees may be applied to past due balances. If you have a problem paying your bill please contact our patient account representative. Payment plans are available if made in advance. We are always willing to work with anyone who shows good faith in meeting financial responsibilities.
Please try to anticipate the need for medication. Refill requests should be made at least one week before your medication runs out. The most expeditious method to obtain a refill is to CALL YOUR PHARMACY FIRST. The pharmacy will submit an electronic request to your physician with all the details required for processing. This will reduce wait time and phone tag between you, our office and the pharmacy. Please allow 48 hours for your request to be processed and then check with your pharmacy.
You will find your test results in the Patient Portal as soon as they are available. A physician will contact you by telephone if there is a concern or abnormal result. Some test results will be reviewed by a follow-up appointment.
Our clinical staff will coordinate and assist you with referral appointments. Please respond promptly to any Patient Portal messages regarding referral appointments.
The patient portal provides easy access to your medical information. From the home page please select the MY HEALTH tab to review or download and print your Visit Summaries, Test Results, Medication Lists, Vitals and Medical History.
